It appears as a basement-like area with a room containing the Trophy Machine. And if you click on the lever, you would get the "Test achievement please ignore" achievement, but the machine is actually broken. But after playing the Epilogue and agreeing to the Settings Person's ideas, you can revisit the Trophy Machine and get the achievement.
